Short-Term Stay
After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient
Residential Stay

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs

Our Center is focused on adding specialty clinical programs to better serve patients. In some cases, additional training and education can be provided for staff on specialized equipment and treatment programing that may enhance recovery times. Some programs can include Stroke, Pulmonary and Post-Acute Cardiac Event Recovery as well as In-House Hemo Dialysis and Telemedicine services.   • Out-Patient Therapy
    Our center offers the availability of Out-Patient therapy for Physical, Occupational and Speech Therapy after an in-patient stay or directly from a patient residing in the community with appropriate physician orders.
  •  In-Patient Hemo Dialysis
    Our Center offers the availability of In-Patient Hemo Dialysis for qualified patients during a Short In-Patient Stay after a hospital admission. Patients requiring this service receive a shorter, daily dialysis treatment allowing them to remain in our center and avoid transferring to an offsite, outpatient dialysis facility.
  •  Post-Acute Cardiac Event Recovery (PACER) Program
    Our center offers the availability of a treatment program focused on in-patients who have suffered an acute cardiac event with a multidisciplinary approach specific to the patients needs and is centered on the patient’s individual pace of recovery.

Post COVID-19 Recovery Programs

Our Recovery Program is designed for individuals who have been diagnosed with COVID-19, required hospitalization, and are now in the recovery phase. We provide a caring environment to meet the rehabilitation and nursing needs to help you return to your prior level of function. To participate, you must have been diagnosed with COVID-19 a minimum of ten days prior to admission and have no new symptoms.

Telemedicine

Our Telemedicine service allows our residents to be examined by their doctors through the use of video conference technology and digital diagnostic tools. This virtual interaction can provide needed medical care and treatment while avoiding a stressful ambulance ride and uncomfortable hospital stay.

I came here to Boulevard sometime in September, I was here about a month. I was discharged on October 15th this year (2024). This place leaves much to be desired. If you need to come here or bring a loved one here just be sure they have an advocate. I pretty much was able to advocate for myself. I was there for 5 days before my medication was given to me, so you must be patient and settle in before you actually receive the care you came there for. One amazing nurse that is really on the ball is Bastia. She resolved so many issues. Marie is really a sweetheart. She is so easy going. I can’t say the same for Paulette. When you are lied to, it’s hard to rebuild trust. I was taking 3 bags of antibiotics daily. It didn’t start until I was there for five days. Bastia straightened that out, as she did other issues as well. I had arrived there from Delray Medical, I had open heart surgery in July. The first rehab after surgery was was The Legacy in Boca Raton. That place was the most God awful nightmare. When the people at Boulevard complained about it. I said, “If you think this place is bad here go to the Legacy.” Then you will love Boulevard. The stop date for my antibiotics was Oct. 13th and I was uncertain if it had been taken into consideration that my treatment didn’t start for the first five days upon my arrival. So when Bastia came in my room. To give it to me she said it was inclusive (meaning the three bags per day for the 13th.) I understood and Bastia went home and I dealt with Paulette. I was told that I was finished with the antibiotics. Paulette became adamant and said to me, “No you are finished there are no more bags of antibiotics left” I said, “Then why did Bastia tell me there were 7 bags left.” The nurse (Mike who I just love) said I can give you the your last few bags today if you want. I said, “ I will be fine I’ll talk to my infectious disease doctor, who said, I was to take all that were sent to me! He ordered the an RX oral ones for me to take at home. You have a gal there that’s a CNA I think named Sophia, and that woman will stop at nothing to be sure you have everything you need. The laundry situation needs a total so over. They ended up loosing a bag of my clothes. I purchased something from Walmart it was Imodium ad and the took it and never gave it back. You can see how many wonderful things there are about this place. After a bit of discord with Paulette, I was told I would be discharged on Oct.15th and that was fine but I never understood why it took 6 people to charge into my room to tell me. All I needed was the charming little pigtailed girl that asked me to sign the paperwork. She’s the sweetest .

This facility is an improvement from the previous skilled nursing facility my mother had been in. The staff is more professional, courteous and responsive. The facility is clean and well maintained. Thanks to all that work there.
