Short-Term Stay
After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient
Residential Stay

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs

Our Center is focused on adding specialty clinical programs to better serve patients. In some cases, additional training and education can be provided for staff on specialized equipment and treatment programing that may enhance recovery times. Some programs can include Stroke, Pulmonary and Post-Acute Cardiac Event Recovery as well as In-House Hemo Dialysis and Telemedicine services.   • Out-Patient Therapy
    Our center offers the availability of Out-Patient therapy for Physical, Occupational and Speech Therapy after an in-patient stay or directly from a patient residing in the community with appropriate physician orders.
  •  In-Patient Hemo Dialysis
    Our Center offers the availability of In-Patient Hemo Dialysis for qualified patients during a Short In-Patient Stay after a hospital admission. Patients requiring this service receive a shorter, daily dialysis treatment allowing them to remain in our center and avoid transferring to an offsite, outpatient dialysis facility.
  •  Post-Acute Cardiac Event Recovery (PACER) Program
    Our center offers the availability of a treatment program focused on in-patients who have suffered an acute cardiac event with a multidisciplinary approach specific to the patients needs and is centered on the patient’s individual pace of recovery.

Post COVID-19 Recovery Programs

Our Recovery Program is designed for individuals who have been diagnosed with COVID-19, required hospitalization, and are now in the recovery phase. We provide a caring environment to meet the rehabilitation and nursing needs to help you return to your prior level of function. To participate, you must have been diagnosed with COVID-19 a minimum of ten days prior to admission and have no new symptoms.

Telemedicine

Our Telemedicine service allows our residents to be examined by their doctors through the use of video conference technology and digital diagnostic tools. This virtual interaction can provide needed medical care and treatment while avoiding a stressful ambulance ride and uncomfortable hospital stay.

I want to start by thanking all the staff at Boulevard for the care and compassion each provided to my mother. My mother was a long term resident at Boulevard for about two and a half years. After two hip replacement, two strokes, many falls and other medical issues, home health care was just not enough anymore. As much as my mother did not want to leave her home, my brother and I had no choice but to seek more care for my mother. She lived up the road from Boulevard and we knew of the place as her husband was a resident there fifteen years prior. My mother was not an easy resident as her mind was sharp, but her health issues continued to take a major toll on her body and mobility. I have seen first hand the compassion and help that the staff provided to and for my mother. Each time I would visit my mother at all times of the week, the place was extremely clean and welcoming. My mothers room was always well kept and many times I would see the staff helping and caring for my mother. Paulette was the Unit Manager on the wing that my mother was staying and she always would provide my brother and I with updates and the status of my mothers condition. She returned phone calls and I have to add spoke with my brother and I with suggestions to help aide us with my mothers conditions. I met many CNA's that cared for my mother and all knew her by name and I always seen them care for my mother with compassion. I had the opportunity to talk with the Administrator named Tim and the Director of Nursing, Tanisha. Every time I would visit they were always out and about and interacting with not only the families, but the residents as well. I have seen them stop and talk with residents like they were life long friends. They new their names and situations and it always made me smile to see the care and compassion they have provided to all. They truly care and it shows. It shows with most of the staff I saw every time I would visit. It is truly refreshing these days to witness a collective group of people care for our loved ones as we would. My mother passed away two weeks ago at the age of 84. I wanted to take a moment to stop and say thank you with my heart to all the staff at Boulevard. To watch a loved relative health fail and life slipping away is one of the hardest things a person has to do in life no matter what your faith my be. Boulevard helped me get through these past couple of difficult years. Thank you Boulevard for your caring a compassion and for providing such care.

But I am blessed because people treat me like their own I am grateful. At Boulevard I have been blessed to meet Allison and Sophie the activities director. For me it would be hard to be pleasant to everyone, but they do a great job. I am grateful because whether I poop Mona changes me. And everyone helps everyone.
